WLAOTC Waivers and Liability Forms including COVID Waivers

Information on the WLAOTC form is required for participation in our classes. It needs to be filled out at least once every calendar year. Checking the box at the end of the form indicates your signature of acceptance of the terms of the waiver. These forms must be filled out by the handler who will be participating in the class. If a second person has been approved to attend, they must fill out and submit a separate set of online forms.

Due to the conditions created by the COVID pandemic, we are required to complete and keep a form by the county parks and recreation department as well as sign and submit a second form that will be sent to them.. Both forms contain waivers based on COVID restrictions. Your checking the boxes on those forms is the equivalent to signing the form. You may be asked for initials and a signature that will be used for a print format. (This feature is still under development) Access to the COVID forms will follow completion of the WLAOTC waiver.

If you have to repeat the first waiver, you will need to send an email to webmaster@wlaotc.com to get the origina information removed. If you need to print the second or third form, click here to be directed to that part of the dialog. You must have completed the WLAOTC waiver for this link to work. WLAOTC Membership Forms

Our membership renewal is due each year on January 1st per our bylaws. Please understand, you must already be a member to use this option. Please click here to renew. Fill in the requested information, and after submitting, select your payment option. You will need to contact the memberhship chair if you require a hardcopy form to renew. If you have questions, please use the Contact Us link for membership dues.

If you would like to join our club, new members will need to fill in the New Membership form. Information requested is solely used for club business and information is only provided to our membership in hard copy format. The document is an Adobe PDFs.
